Liability Release

Participation in camps and tryouts involves risks, including falls, collisions, contact with other participants, and other sports-related injuries. By agreeing below, the participant and parent or legal guardian acknowledge these risks and release Chattanooga Kaistorms FC, its coaches, staff, volunteers, and facility partners from claims arising from ordinary risks of participation, to the extent permitted by law. This release does not waive rights that cannot legally be waived.

Treatment of Injury

If the participant is injured or becomes ill during a camp or tryout, I authorize Chattanooga Kaistorms FC staff to provide reasonable first aid and to contact the parent, guardian, emergency contact, or emergency medical services when appropriate. I understand that medical expenses are the responsibility of the participant or parent or legal guardian, and that this authorization does not replace professional medical advice.

Picture and Video Release

I give Chattanooga Kaistorms FC permission to photograph or record the participant during camps, tryouts, practices, or related activities and to use those images or recordings for team, community, website, social media, and promotional purposes. I understand that I will not receive payment for these uses. I may contact the organization to request that future use stop, where reasonably possible.
